Position Summary

Works under general supervision to perform in-take enrollment assessments within a four county service area; schedules and conducts in-home face-to-face visit to assess potential applicant’s needs for extensive in-home services, as well as the applicant’s natural support system, as part of the application processes for enrollment into CHOICES; contacts consumers within 24 hours of receipt of assigned referral to schedule in home face-to-face visit. Visits must be conducted within 5 business days of receipt.

Minimum Qualifications

100% -1. Two (2) years of experience in case management, social work, or nursing; and graduation from an accredited college or university with a bachelor’s degree in social work, sociology, nursing, or related field; OR

2. Three (3) years of experience in case management, social work, or nursing; and graduation from an accredited college with an associate’s degree in social work, sociology, nursing, or closely related field; OR

3. Four (4) years of experience in nursing; and graduation from an accredited nursing program with a diploma.

4. PROOF OF EDUCATION, TRAINING, AND/OR EXPERIENCE IS REQUIRED.

Duties and Responsibilities

1. Receives referrals from(Single Point of Entry) SPOE on potential non-Medicaid applicants or MDS Section Q referrals, staff must conduct initial and follow-up functional and social assessments on older adults and adults with disabilities. A standardized statewide tool is utilized to determine whether the applicant appears to meet Choices enrollment criteria.

2. Counsels with potential enrollees and family members about the CHOICES program which includes explanation of the CHOICES program, who qualifies for CHOICES, limits on enrollment, Federal Estate Recovery Program. The family/caregiver/representative must have an opportunity to ask questions regarding information provided. In-take staff should be able to explain on a level of a 5th grader. Patient Liability Acknowledgement form must be explained and signed as well.

3. Facilitates the Medicaid Application during the intake visit; must collect, scan and or copy in the home all appropriate DHS required documents and identifications.

4. Prepares initial documents and chart prior to making in-home visit which includes completing the Collateral and Applicant HCBS Tools.

5. Facilitates and start the PAE (pre-admission evaluation) application process electronically in the home on laptop by logging into the online TPAES systems.

6. Maintains individual caseloads and reports and give weekly updates to management of status until consumer is enrolled in the program.

7. Documents in progress note action to a consumer’s file.

8. Closes out file for consumers who do not complete the process also for consumers whose required documents are not received.

9. Performs other related duties as assigned or directed.

KSAs and Others -1. Knowledge and skilled in the use of Microsoft Office Suites (Word, Excel, etc.)

2. Ability to plan, organizes, and maintains an even flow of work.

3. Ability to spell, punctuates, and uses grammar, correctly.

4. Ability to analyze and solve problems and exercise good judgment; and to express ideas clearly, concisely, and effectively verbally and in writing.

5. Ability to communicate courteously with the public, other departments, and to resolve problem situations with results.

6. Knowledge of the aims and purposes of the assigned agency.

Special Requirements

1. Registered or Licensed Practical Nurse or Licensed Social Worker

2. Must possess a valid Tennessee driver’s license or secure one as of employment date

3. Must have daily access to an operable automobile and meet County requirements for Automobile insurance upon date of employment

4. Must pass background check to include: Tennessee Abuse Registry, Tennessee Felony Offender Registry and the National and Tennessee Sexual Offender Registry.
